Privacy Collection Notice

1. What is in this Notice?

This notice describes how we collect, hold, use and share your personal information. It includes:

  • the kinds of personal information we collect;
  • the reasons why we collect and use that information;
  • who we share your personal information with;
  • the consequences if we are unable to collect your personal information; and
  • how to access our Privacy Policy or contact us about our privacy practices.

2. What is Personal Information?

Personal information includes information or an opinion about an individual that identifies them, or that can reasonably be used to identify them. This can include a person's name, age, gender, employment history, health information and contact details.

3. What Information We Collect

We collect personal information about you when you engage with us directly or via the Flourish 360 platform. We may also collect personal information about you from your employer.

The personal information we may collect includes but is not limited to:

  • your name;
  • your email address (usually your work email address);
  • your year of birth;
  • your gender;
  • the location where you work for your employer;
  • details about your job role or function;
  • the department you work in; and
  • details about your perception of your workplace's psychological health, safety and wellbeing.

If you give us information about other people, you must have their authority to do so and must tell them what is in this notice.

4. Why is Your Personal Information Being Collected?

The purpose of collecting your personal information is:

  • to confirm your identity and create an account with the Flourish 360 platform;
  • to manage our relationship with you, including contacting you if you need IT support when using our platform; and
  • to undertake an assessment of your perception of the psychosocial hazards in your current workplace.

5. What Would Happen if Your Personal Information Was Not Collected?

If you do not provide, or if we are unable to collect, your personal information, or the information you give us is inaccurate or incomplete, we may not be able to create an account for you to use our platform, and we may not be able to undertake an assessment of your perception of the psychosocial hazards in your current workplace.

6. Who We Share Your Personal Information With

The personal information we hold about you will not be shared with any third parties, including your employer. We will only share de-identified, aggregated reports from the psychosocial hazard assessments you and other employees have participated in.

We do not share your personal information with any third party, and we do not share your personal information with any recipient located outside Australia.
